  • Online Self Referral

At Cavendish Cancer Care we support local people who are living with cancer.

That includes people with a current or previous cancer diagnosis but also family members or carers of people with cancer. We provide individually tailored support to help alleviate the emotional impact of cancer, and the physical side effects of the illness and its treatment. Support is easily accessible and is provided online, by telephone and in person. We are a registered charity, and all of our services are provided free of charge.

We are a self-referral service, so you don’t need to be referred by a healthcare professional, just fill in the short form below. We will then arrange an initial telephone appointment with one of our Cavendish Care Coordinators. This call will provide an opportunity to gather further information about your situation and tell you more about our range of services and how we can best support you.

If you are unable to complete the form online you can call us on 0114 278 4600 and leave a message with your contact details.
